Missing heading inside configure fields popup

Issue Summary

Missing heading element inside configure fields popup.

Steps to Reproduce

1. Navigate to "Create" button in header & activate it.
2. Navigate to "Configure Fields" buttons & activate it.
3. Observe that there is no heading present inside the popup.

Actual Results

Currently there are no headings present inside the configure fields popup so screen reader users find it difficult to understand the structure of the popup.

Expected Results

Provide a visual <h3> heading element with text "Configure fields" inside the popup and provide unique id to it & reference it to the dialog element via aria-labelledby attribute.

Workaround

Currently there is no known workaround for this behavior. A workaround will be added here when available
