Description
Issue Summary
Grouping of form controls used for the input fields inappropriately in the modal dialog
Steps to Reproduce
- Navigate to the "Projects" dropdown and activate it.
- Select the "Scrum" project and activate it.
- Navigate to the "Side navigation".
- Navigate to the "Reports" link and activate it.
- Navigate to the "Switch Report" dropdown.
- Select the report "Burndown Chart".
- Observe that "Burndown Chart" page has been appeared on the screen.
- Inspect the code and check that grouping used inappropriately.

Actual Results
The <fieldset> element is used to define "Project name", "Project key", and "Project lead" input fields in "New Project with board" modal dialog. As a result, screen reader users might get confused
Expected Results
Remove <fieldset> element from the source code of the form fields.
