Issue Summary

In activity stream, the attachment images doesn’t contain alternate text.

Steps to Reproduce

1. Navigate to the "System Dashboard" page.
2. Navigate to the attachment images present inside "Activity stream" section.
3. Inspect the code of the images & observe that alt text is missing.

Actual Results

With screen reader when user navigate to the attachment image link present inside "Activity Stream" section the screen reader doesn’t announce any alt text for the image.

This makes screen reader users difficult to understand the image link purpose effectively.

Expected Results

The attachment images should have clear & descriptive alternate text. 

Since these images can be uploaded by user, while uploading the user should be able to provide alt text for the images & same should be set to the images via "alt" attribute.

Workaround

If user doesn’t provide any alt text while uploading the image, provide a default alt text.

For Example: alt="Screenshot {file-name}"
