Add Issue ID to form response for Jira Issue Collector

      At present (Jira 8.5.x, Jira 8.13.x), installation of the Jira Issue Collector on to a webpage will render the following default message when form is submitted successfully:

      Thank you for your feedback!

      Thanks for providing your feedback! This window will automatically close in 5 seconds.

      It would be ideal if this message could instead include the Issue ID for the Issue that was just created, so the user is immediately aware of an ID they can track.
