Bulk Create issues (JIM) doesn't allow importing into non-default issue types

The Bulk Create issues menu ("Import Issues from CSV" on the issues menu) does not allow users to map a column in the source file to an issue type. (JIM 7.0.12)

The net result is that the Bulk Create issues menu is only capable of importing issues into a single default issue type. To add insult to injury, the error messages in the import log repeatedly complain about the lack of an issue type in the import file and assert that every issue will only use the default.

INFO - Issue does not have issue type selected, the affected issues will be created with default issue type [Bug]

While this may have been omitted to prevent non-admin users from spuriously creating new issue types, it nonetheless results in a feature that is usable in only the narrowest set of conditions.
