Remove the survey questions from the setup process on stable

During the JIRA setup process on stable we have a screen that attempts to capture some info through use of survey questions.
These aren't needed anymore, so we need to get rid of them.

This incorporates a couple of things:

• remove the call to get the questions from hamlet
• remove the submission of the question responses to hamlet
• remove any code processing the questions, checkboxes etc

This code lives in the setup-license.js file I believe.
