Feedback dialog no longer renders correctly.

      1. (BUG) Click the feedback link in the JIRA footer.

      • Too many scrollbars. The outer scrollbar scrolls the whole dialog.
      • The name field is hidden unless you scroll to be bottom of both scrollbars.
      • The e-mail fields is just hidden (you can't see it). This leaves people raising feedback without ever giving a way to contact them.
