View Custom Fields shows the same custom field twice

It's possible to have a custom field listed twice (the same custom field with the same id).

To reproduce you need to restart JIRA and create a custom field before custom fields cache was initialized (so you must not use View Custom Fields page before you create a custom field).

Found when running integration tests which create a custom field right after JIRA restore was run.
