Steps to reproduce:
1. Open the Create Issue (or Subtask) popup
2. Add a summary
3. Change the Issue Type

If you submit the form now your issue will be created but without the attachment because performing step 3 causes all the checkboxes under the Attachment field to be unchecked, resulting in an issue created but annoyingly without any of the attachments you uploaded.
