Allow post functions to change values of Custom Fields

      Currently the post functions on a transition only allow certain pre-configured fields to have their values changed.

      This functionality should be made available for custom fields as well, since they are equally likely to need to be changed on a transition e.g. automatically cleared on Re-open.
