Change initial issue-type to a "please choose" type.

When a user first selects "create issue", its too easy to just click through leaving the issue type at the default item. What would be good is a having a "please select" as the first item, and some javascript check to ensure that the user has actually selected a valid issue type before continuing.

Successive create-issues would use the cached issue type as current behaviour.
