Bugzilla has a nice feature whereby existing attachments can be marked as superseded when adding a new attachment.

The superseded attachments are not removed, but they are shown in a strike-through font.

The old attachments can still be downloaded (which can be very useful at times), but it is very obvious that they are current.

Very simple, but effective.
