Add information about default issue type scheme to user documentation

The current documentation doesn't tell you that if you create a custom issue type scheme for a specific project, all of the new issue types will be associated with every project that uses the Default scheme.

The docs might be amended to say that you should not use the default issue scheme if you are going to create issue types that only apply to a specific project.
