Add ability to comment and auto-link to attachment when using Attach File operation

When a user clicks on the Attach File operation, there should also be a comment automatically added with a link to that attachment. And if the user adds a comment at the same time as attaching, the link should also be automatically created beneath the user's comment.

Also, the manage attachments grid should also be shown on the issue view page, for a better overview of all attachments.
