When Creating Issue, if form has error, lose attachment value

    Description

      When the user is creating an issue, they hit Submit, if there is an error on the form (i.e. required field not filled out), the form posts back with the error highlighted, and keeps all the previously filled out values, except for the attachment field. The user must then Browse to their attachment again before submitting issue.
