email template for issue created needs cleanup

The email should not include fields that were not specified (eg, if environment is left blank, don't mention it, if no fixfor is chosen ,don't mention it). Also dates are just java date format, which si pretty ugly. Should use a more useful dateformat (no need for sec/ms!). Also, should have a checkbox whether the originator wants to be mailed on the issue. Eg, I just created an issue, so getting an email about it is a bit pointless as I already know about it!
