if an attribute contains the separator, the attribute must be surrounded by quotes ;
if an attribute contains a carriage return character, the attribute must be surrounded by quotes ;
if an attribute contains a quote, the attribute must be surrounded by quotes and the quotes must be escaped by a quote.
