Fix userassociation table to increase source_name column size

      Summary

      The user performed a Server merge of two Cloud instances. In this merge, a Service Desk customer was made a Jira User.

      Problem

      When transitioning issues, the user is receiving a PSQL error such as:

      Caught PSQLException for select "O_S_WORKFLOW_ENTRY"."id", "O_S_WORKFLOW_ENTRY"."name", "O_S_WORKFLOW_ENTRY"."initialized", "O_S_WORKFLOW_ENTRY"."state" from "public"."os_wfentry" "O_S_WORKFLOW_ENTRY" where "O_S_WORKFLOW_ENTRY"."id" = ? limit ?
