I tried to create a new status to go along with some workflow changes and was unable to do so. When trying to create the status I was taken to a 500 error page stating that creating the status was violating a pk uniqueness constraint in the DB (error report attached).

Steps to Reproduce:
1. Login to JIRA OnDemand instance as an admin
2. Go to the Status Administration page
3. Attempt to create a new status

Expected Behavior: Status is successfully created, it is use-able in new workflows, a general sense of happiness and productivity is experienced by all involved parties.

Obtained Behavior: After hitting the "Add" button I am immediately taken to an error page that produces the attached error log
