Preview of attached images fails in 'Create/Edit' dialogs.

      Steps:

      1. Create/Edit an issue.
      2. Attach an image file. E.g. xyz.png
      3. In the Description field, write
      4. Click on the preview icon of the Description field.

      Expected Result:
      Preview must display the image.

      Actual Result:
      Fails on Edit with the following error:

      Fails on Create with the following error:

      Once the issue is edited/created, the resultant issue shows the image correctly on the view issue page.
