Hello Folks!

Trying to set up a custom JIRA Issue Collector at present. Having difficulty with the form loading - the dialog comes up, the trigger works, but there is no content in the dialog.

Funny thing is, if I use the Raise a Bug or Feedback defaults it works. The only things I have changes on the custom one is the title.

This is in OnDemand, so 6.x-something?
