Looking at standard attachment url http://myserver/secure/attachment/16045/myimage.jpg

If I go into manage attachments I have the option to delete this attachment. I can then upload an updated one. The issue is if I have this url reference anywhere it is then a dead link.

There should be away to update a loaded attachment without chnaging the url. This is available in other system,d such as MediaWiki.
