[JOPSCLOUD-49] Fields for incident roles: incident manager, technical lead and comms lead

We should include fields for the most common incident roles, so incident managers can assign them out and clearly track them on the issue.

Current status

This is in our plans for our 1.0, and shown in our mockups, but hasn't been built yet.
