Track postmortems as issues instead of pages

Our initial implementation of postmortems (see JOPSCLOUD-39) will create a Confluence page for teams to fill out collaboratively, and create actions at the end.

Some customers have indicated they'd prefer to create a Jira issue to track this instead, and complete the information on the Jira ticket and link follow-up actions from here.
