Problem
The ProForma Lite "Issue Forms" button/link no longer works in Jira 9.17.1 with ProForma Lite 8.8.4-DC after upgrade or no longer appears after creating a Form for a Project.
Environment
Jira 9.17.1
Steps to Reproduce
1. Install the ProForma Lite: Forms for Jira (8.8.4-DC or later) app.
2. Navigate to Jira Administration>Manage Apps>Forms
3. Create a Form for an existing Project
4. Navigate to the Project used in Step 3 Projects>Project Name
BUG:
Notice that the "Issue Forms" button/link does not appear as it used to form older versions (e.g., Jira 9.8.2). If the form was created for a Project before upgrading to Jira 9.17.1 then the button/link would appear but is no longer functional or leads to a blank page.
Expected Results
"Issue Forms" button/link to the individual forms for the Projects
Actual Results
The Button does not appear or is no longer functional.
Workaround
The Customer can use direct links to the forms.
Notes
The Button will show up for Kaban Projects but will not be functional

FYI, can confirm this behavior still happens in Jira DC 10.3.2 and ProForma Lite 10.3.2-DC 
Only seems to be affecting ProForma Lite and is still present on the ProForma Lite 8.8.5
Add product tag since it is a JSM DC plugin and it is not cross product plugin.
The Issue Forms option exists for me in the left nav, just doesn't work. 😅
Looking forward to a resolution for this! 🙌
FYI, can confirm this happens in Jira DC 10.4.0 and ProForma Lite 10.3.2-DC. (Not a big surprise, as ProForma Lite isn't listed as compatible with Jira 10.4...)
