Drop support for PostgreSQL 11 & 12 in the next major Fisheye/Crucible release, which is 4.9.0.

Notes:

• Fisheye/Crucible 4.7.x supports PostgreSQL 9.6-11,
• Fisheye/Crucible 4.8.9+ supports PostgreSQL 9.6-14,
• Fisheye/Crucible 4.9.0 would support PostgreSQL 13 and newer

Thus, the upgrade path looks like follows:

• Fisheye/Crucible 4.7.x & PostgreSQL 11 -> Fisheye/Crucible 4.8.x
• Fisheye/Crucible 4.8.x & PostgreSQL 11 -> PostgreSQL 14
• Fisheye/Crucible 4.8.x & PostgreSQL 14 -> Fisheye/Crucible 4.9.x
