Drop support for SQL Server 2016

      Fisheye/Crucible 4.8.x already supports newer versions - 2017 & 2019.

      Fisheye/Crucible 4.7.x supports MSSQL up to 2017, so there's an 4.7.x -> 4.8.x upgrade path without updating the database.

      Drop support for MSSQL 2016 in the next major release, which is 4.9.0.
