Fisheye fails to start if the FISHEYE_INST does not contain a config.xml

    Description

      Fisheye fails to start if the FISHEYE_INST does not contain a config.xml. In this case, we should output/copy in a default config.xml on startup and proceed.

      If Fisheye's FISHEYE_INST directory does not exist, it will be created, and then Fisheye will fail to start.
