The small icons next the revisions on activity streams are incorrect - see screenshot.

In my brief investigation, it seems this is two bugs.

1. p4 changes are not setting the correct style for a modified revision. So that is one issue.
2. the default icon is wrong (it is a changeset icon) and not aligned
