Allow mapping of custom attributes from CSV

Currently, if a user wants to import users using CSV into their Crowd instance, they are unable to map any custom attributes (such as phone number). This can be somewhat of a hindrance, especially when the user would like to import a high amount at once into Crowd (along with their custom attributes).

It would be great if custom attributes could be mapped into Crowd as well.
