CrowdHttpAuthenticatorImpl should use the CrowdClient to determine the SSO cookie name

      Since Crowd 2.1, we've had the ability to retrieve the SSO cookie name using the CrowdClient - we should do it. The cookie name should be overridable from the crowd.properties file.
