Customer suggested following improvements when creating new crucible review:

It makes more sense if the "Author and Moderator of the created Review"
is the person who requests the review (the reporter of the jira issue
who clicks on "request review code"). The person who is assigned the
review is then added as a reviewer to the review, so in their dashboard,
the reviewer participant would see a review appear in their "To Review"
filter. Ideally once the assignee (reviewer) or reporter
(author/moderator) resolves the "In Review" jira ticket, the crucible
ticket is automatically summarized. If the author/moderator closes and
summarizes the crucible ticket, it should also close the jira ticket as
"fixed and reviewed". That way there is a tight relationship between
the jira issue and crucible review, and the crucible and jira roles are
aligned. Make sense?

If I were to include the field additional assignees to the workflow
screen, could you also add these as additional reviewers to the crucible
ticket?
