Summary
When a page containing images is copied to a different page without "include attached files and images" and if a new image is added to this copied page, the new image is duplicated in place of all the existing image placeholders.

Environment
Tested on Confluence 8.5.0
.
Steps to Reproduce
1. Create a new page and add three images (A,B,C) and save the page:

2. Copy this page without checking the "include attached files and images" option so that a new page is created. This new page will have the three unknown attachments which is expected. Save the page.
3. Now edit this copied page and add a 4th attachment (D) to this page and save.
Expected Results
After Step 3, only the 4th attachment should be visible along with three unknown attachments placeholders.
Actual Results
There are 4 attachments displayed in the page and all 4 of them are "D"
Workaround
No workarounds at this point.
