Steps to show unintuitive behavior:

1. Click add comment
2. Click insert/edit image button
3. Define file name
4. Check thumbnail
5. Change alignment to centre
6. Click browse next to attach file and choose an image file
7. Forget to click attach
8. Click ok

This produces a broken link. However- it should notice that the user forgot to click attach and confirm that before adding the comment. Or some other mechanism to keep the user from adding broken links.
