Confluence should allow for virus/malware scanning of attachments, which could then be rejected if viruses/malware were found.
In addition to the viruses/malware scan, while XSS attachments are blocked from running within Confluence, it would still be beneficial to also scan for any XSS findings to prevent users from downloading these files as well.
