Confluence Table Of Contents Macro is incompatible with Refined for Confluence theme

Steps to Reproduce

1. Install refined for Confluence
2. Add a heading to the sidebar
3. Add a Table of Contents macro to a page

Expected Results

Sidebar headings should not be picked up by ToC macro. Only headings from page content should be included.

Actual Results

Sidebar headings are shown within ToC macro.
