Captcha is not shown on reply-to comment form

      Steps to reproduce:

      1. Enable captcha.
      2. Find a comment on a page/blog post.
      3. Click 'Reply to comment'.
      4. Bug! No captcha displayed.
      5. Enter something, hit Post.
      6. Bug! Error message appears: captcha not filled in correctly. There was no captcha shown to the user. Captcha is shown at this point, and can be entered successfully.
