A workaround to escape emoticons in Confluence would be to create a user macro which only contains the body. Therefore the content wouldn't get rendered. However, when changing between Markup and Rich Text mode, the Rich Text editor adds a paragraph around the macro and therefore makes it impossible to have inline macros.
Example:
Markup Mode
Inline
{pre}![](/images/icons/emoticons/wink.png)
Text
> Rich Text:
Inline
{pre}![](/images/icons/emoticons/wink.png)
Text
> Markup:
Inline
{pre}![](/images/icons/emoticons/wink.png)
