The Knowledge Base Survey Plugin that is used by Atlassian KB articles is no longer publicly available and it seems to be only for internal use within Atlassian projects for now.
It is not clear why the plugin was taken down by Atlassian, however, it would be of a great addition to the Confluence offerings if it is revived back and supports all the latest Confluence versions.
