      I'd suggest to implement X-Accel-Redirect/X-Sendfile functionality for confluence.
      There can be an option to pass request to confluence for attachment download with special flag or verb to get X-Accel-Redirect/X-Sendfile path (relative to confluence data attachements folder) to be served by HTTPD server (apache, nginx, lighthttpd, ) in front of confluence.
