As part of CONF-21909, an additional index was added to the FOLLOW_CONNECTIONS table. This has broken the in-place upgrade job of the Oracle 11 and 12 builds, since the schema upgrade process tries to add an index that is already present.
This is being initially marked as a blocker for OD-29, since while the bug doesn't seem to affect Postgres, the fix may involve removing the additional index from the upgrade process, and so it'd be best if this didn't make it on to OnDemand to start with.
