Two different Heading-4 lines have different font sizes

I have a document with two sections, both which use heading-4. The two lines are clearly using different fint sizes. This appears on both Linux and Win7.

The first image shows the page in viewing mode. The second two show the page in edit mode, with a word from each line highlighted to show that "Heading 4" is the type.
