I haven't seen this on my install, but I don't have captcha turned on. I don't know if this is an issue with Confluence itself or just with Atlassian's installation that they use for the confluence docs.
My client environment is IE Firefox 16.0 (Mozilla/5.0 (Windows NT 6.1; WOW64; rv:16.0) Gecko/20100101 Firefox/16.0) on Windows Server 2008 R2 Standard SP 1.

Description
When you post a comment to a page in the Confluence documentation, is has a captcha that you have to enter, along with "Preview >", "Save" and "Cancel" buttons/links. If I enter the captcah incorrectly, I get a red pop-up telling me the captcha check failed, and the "Save" button text has changed the "publish.name". See attached screen shot.
