After attaching a file (tried with pdf, ascii text fiels, docs) that is correctly indexed by lucene (the document was found when providing words in the search mask) i have changed (edit & save) the wiki page where the file is attached.

After this step i inserted the same searchterm than before in the search mask, at this time the attachement isn't in the search hits any longer.

A few minutes ago i have tried it on the current Confluence test instance and got the same result (see http://confluence.atlassian.com/display/TEST/brads+test), so this has nothing to do with my specific setup.
