Issue Summary

When uploading files, the first file uploads fine, but when the user goes to select a second, different file (even from a different file location), the system uploads the first file a second time. 

Steps to Reproduce

1. In a work item in comment or description section upload a PDF file
2. Upload a second pdf

Expected Results

Both pdf should be uploaded

Actual Results

The second file starts loading and then onces it is uploaded it appears to be the same files as the first one uploaded 

Workaround

You need to wait a few seconds until first file completely upload or refresh the work item a few times and try to reupload the files.
