Issue Summary

In some space-level PDF exports, the table of contents that get included by default is sometimes missing but can be found at the very end of the PDF file.

This seems to happen on very specific page trees when performing a custom PDF export but it is not clear at the moment what the pre-requisites are.

Steps to Reproduce

1. Perform a custom space PDF export on an affected page tree

Expected Results

The table of contents is included 

Actual Results

The table of contents is included but the first page of the table can be found at the very end of the resulting PDF file

Workaround

Manually edit the PDF file to rearrange the pages
