Ability to mandate page version comments on publish

Currently, publishing with a version comment is relatively out of the way (in the menu, not done by the big 'Publish' button) which leads to users ignoring it.

Making the version comment more front-and-center, with the ability to block publishing when a comment is missing/blank, would help make the page history much more useful.
