This should work like Google Docs.  If a user is viewing a page and another user makes a comment on the page you should not have to reload the page to see the inline comment.  This should work like this:

1. User highlights text and clicks add comment
2. User hits save on the comment
3. Anyone viewing the page now sees the highlighted text
