Use macroHash when no macroId is available

      Steps to Reproduce

      1. Create a page with a macro on Confluence 5.7 or earlier (before the introduction of macro ID's)
      2. Upgrade to use atlassian-connect-plugin 1.1.32

      Actual Results
      The values of the macro.hash and macro.id context parameters are both empty.

      Workaround
      Edit and save the page. This will cause a macro ID to be generated.
