Macros from the Advanced Tables for Confluence add-on don't work initially when a new customer signs up for a Confluence Only instance and chooses the Advanced Tables add-on during the sign up process.

Steps to Replicate
1. Sign up for a new OnDemand evaluation at https://www.atlassian.com/ondemand/signup/
2. Select confluence and advanced tables for Confluence on the initial configuration screen.
3. Wait for your instance to be provisioned.
4. Login and try to add the

{table-plus}, {csv}, {json-table} or {attachment-table}.

to a page.
5. The plugin cannot be enabled via the manage add-ons menu in OnDemand.
